Piles or hemorrhoids are nothing but swollen and inflamed veins present inside the internal regions of the penis or the rectum. As a result there may be simmering pain whenever there is a strain on that particular part which is infected. There might be several reasons for the development of enlarged veins. One of the reasons being the flow of blood through the veins slowing down or due to any obstruction caused similar to the case of varicose veins in the legs. Another theory is that hemorrhoids are caused due to very high pressure in the veins which is humans tend to remain upright unlike other species. However some people might have more of a tendency to suffer from hemorrhoids as compared to others. 

Some of the common reasons considered to be a factor for hemorrhoids include
1. Genetic factors 
2. Pregnancy 
3. Age 
4. Obesity

There are two types of hemorrhoids depending on the place where it is located
1. Internal hemorrhoids: internal hemorrhoids are the most difficult kind of hemorrhoids to detect because they are not visible to the naked eye .also they are relatively painless. The only significant difference might be found during the emission of stool. This is because strain caused by emission of stool might lead to injury on the delicate surface of the hemorrhoids causing it to bleed. As a result of this you might notice red blood on your toilet tissue or in the toilet bowl 


2. External hemorrhoids: this kind of hemorrhoids are far more painful than the internal hemorrhoids this is because in internal anal membranes lack pain sensitive nerve fibers but our external surface has a very sensitive pain nerve fiber. Often blood may pool in an external hemorrhoid and form a clot (thrombus), causing severe pain, swelling and inflammation.



 

Treatment
Treatment of hemorrhoids is at different levels .treatment depends on varied factors like the intensity of the complication and also the place where the hemorrhoids is. If the pain is very nominal you can treat it with over the counter medications, ointments or pads containing some kind of anti inflammatory agent along with daily warm baths. But if the pain is too much of a problem and you are not being able to treat it with off the counter medications you might need to see a doctor and might even need a surgery. If there is a blood clot in an external hemorrhoid it can easily be removed with a simple incision which provides for instant relief. 

 

Some common procedures include
Banding hemorrhoids: in this method the doctor places one or two rubber bands along the base of the internal hemorrhoid in order to cut off the circulation. As a result sooner or later hemorrhoids fall off. This simple procedure — called rubber band legation — is done in the doctor's office and is effective for many people. 

Sclerotherapy: In this method a chemical solution is injected into the blood vessel to shrink the hemorrhoid so that it can easily be removed after that.

Infrared light: Here the doctor uses a 1- or 2-second burst of infrared light which cuts off circulation to small, bleeding, internal hemorrhoids. 

Surgery: Another most critical cases when you have very large hemorrhoids you might need to have a surgery where the doctor removes the whole tissue in a process called Hemorrhoidectomy. Normal this surgery is done using local anesthetic combined with sedation, a spinal anesthetic, or general anesthetic. It's normally requires an outpatient or overnight hospital stay. Although the surgery might be painful it is the most comprehensive method and medicines might be used to decrease the pain.
